Being sick is a normal part of childhood, and being seriously ill is the unfortunate lot of many children. Every child in the United States has some contact with the healthcare system at some time, and it is estimated that one of every two children or adolescents is hospitalized as a result of illness or injury. Being injured, undergoing routine medical procedures, getting sick, or being hospitalized confront children with challenges on many levels--physical, mental, emotional, and social. The premise of this volume is that developmental and psychological factors are central elements in many current problems in child health such as persistent crying in infants, sources of children's injury and respiratory illness, children's coping with medical procedures, childhood trauma, and physical and mental well-being in adolescence. Understanding, promoting, and maintaining children's health, therefore, depend to a great extent--and are likely to depend even more in the future--on elucidating the determinants and consequences of children's and parents' health-related behaviors and attitudes. Child/adolescent development and behavior have been a traditional "concern" of prima ry health care providers. However, it was not until the mid-1960s that attempts were made to consolidate developmental-behavioral issues into an identifiably distinct fund of medi cal knowledge. During the ensuing two decades, developmental-behavioral pediatrics was recognized as a clinical and research subspecialty, within the framework of compre hensive health care for children. The influence of public advocacy groups, topic-dedicated journals, national professional specialty societies, subject-related continuing education programs, and federal legislation (PL94-142) has served to crystallize developmen tal-behavioral pediatrics as a specialized field of study. As a consequence, during the past ten years significant modifications have restructured medical student and pediatric resi dent education, providing an emphasis on developmental-behavioral issues. The focus on neurodevelopmental, educational, and psychosocial issues reflects changing priorities in traditional health care for children. The postgraduate training of pediatric fellows, in two and three-year training programs, was initiated to accommodate professional manpower needs in both academic and practice settings. Many of the problems in childhood development and behavior frequently span the traditional areas of child neurology, child psychiatry, and general pediatrics. As a result there has been some confusion in demarcating professional responsibilities in diagnosis and management, as well as poorly defined terminology and classification schemas. With the birth of developmental pediatrics as a pediatric specialty, a more cohesive fund of knowledge has been accumulated and more meaningful strategies have been designed for prevention, diagnosis, and management.

Increasingly more and more children with developmental disabilities survive into adulthood. Pediatricians and other clinicians are called upon to care for an increasing number of children with developmental disabilities in their practice and thus there is a need for a practical guide specifically written for paediatricians and primary care clinicians that addresses major concepts of neurodevelopmental pediatrics. In the United States, the specialty training leading to a conjoint board certification by the American Board of Pediatrics and American Board of Psychiatry and Neurology, requires a total of 6 years of training (2 years of pediatrics, 1 year of neurology, 18 months of child neurology, 18 months of neurodevelopmental disabilities). As of December 2006, in the US, there were 241 pediatricians and 55 child neurologists certified in the subspecialty of Neurodevelopmental Disabilities. Thus most of the children with developmental disabilities are seen by pediatricians and therefore it is important for these pediatricians to be well informed of common issues in the field. This volume initiates a series devoted to theory and research in behavioral pedi atrics. Focusing attention on the limited scientific contributions to understanding the development of behavior will, we hope, stimulate further research in this vitally important area of human knowledge. The ability of an individual to achieve his or her full potential as an adult is to a significant degree determined by critical variables in this development within a given society. Study of this aspect of human biology, therefore, may have far-reaching effects on the evolving human species. An awareness of the complexity of the behavioral patterns in infancy is of relatively recent origin and is an obvious essential starting point for this series of publications. The need to discriminate between objective observations and theo retical constructs and to design scientifically valid experiments is vital to progress in understanding early infant behavior. It is easy to misinterpret apparent responses to interventions, especially in infants at biological risk. Preterm infants and their caretakers are particularly challenging as subjects but very difficult to assess scientifically. The interactions between mother and newborn and father and newborn hold promise of substantial additional insights into the development of behavior.
