The lamp is placed behind and a little to one side of the eye to be examined. In order to see the optic disk, the patient is told to look at the ear of the observer on the side opposite to the eye being examined. In this way the axis of vision is directed inward, and the optic disk readily brought into view. These examinations are made in a room lighted only by the lamp used in the processes. It is sometimes necessary to dilate the pupil with atropia, in order to obtain a view of the disk, but experience and tact will generally enable the observer to dispense with this rather dio agreeable procedure. Excerpt from A Clinical Treatise on the Diseases of the Nervous System The erudition displayed in Dr. Rosenthal's treatise does not recog nize the boundaries of country, though it is but natural that the author should chiefly refer to the works of his compatriots. We will find in this work, which is not deficient in original observations, a concise but complete expose of important researches upon various questions in nervous pathology by L. Tuerck, Benedikt, Meynert, and other authors of the Vienna School. Among works of this character, which are, in my opinion, too little known among us, I shall restrict myself to calling attention to those which discuss the anatomical lesions of the nervous centres in hydrophobia, the lesions of the arteries in cere bral syphilis, and, finally, the observations which have established, according to Dr. Voigt, the relations of cutaneous anaesthesia and hyperaesthesia with the distribution of the nerves to the integument. The study of foreign works upon nervous diseases has led me to pay attention to certain studies in comparative pathology, which may be applied to the investigation of the changes which morbid types of this class experience, according to climate, nationality, race, etc., but without losing their autonomy. As a rule, we are only able to point out slight shades of difference in a study of this character. But the deviation may sometimes be characterized by more or less profound modifications, even in adjacent countries and under very similar parallels of latitude. Excerpt from Clinical Treatise on the Diseases of the Nervous System, Vol. 2 The morbid processes occurring in the cortical cells of the brain in general progressive paralysis may also develop in the cord under the influence of various acute or chronic inflammatory affections. Degeneration and pigmentary atrophy occur in the multipolar nerve-cells of the anterior horns and in the fibres which unite these cells with the intra-medullary portion of the anterior roots and with the antero-lateral columns. From careful study of the affections of the anterior spinal nerve-roots we have become better acquainted with the influence of these delicate and important parts upon the motor and trophic functions, and a whole series of morbid types have been discovered which may be combined with various bulbar or spinal paralyses. The anatomical data have so much the more importance with regard to the theory and clinical knowledge of the diseases in question, because nature herself, by isolating the lesions in certain nerve-cells, in certain groups of cells, or in certain bundles of the white substance, has furnished us with a positive and precise solution of all the problems which could be determined by experimentation. The most skilful and practised experimenter could never obtain equally positive results. In the following section we shall first examine the rapid inflammatory processes occurring in the anterior horns (myelitis antica acuta) as they are found in acute spinal infantile paralysis and in that of adults; then, the chronic inflammatory processes in the anterior gray columns (myelitis antica chronica), which constitute the fundamental lesion of progressive muscular atrophy and its different forms.
